Welcome Message

“Together Towards Better Lives.. Despite the Challenge”
Distinguished Guests,
Esteemed Doctors, Researchers, Nurses, Patients, and Policymakers,
Welcome to the Chronic Diseases Conference 2025,
Held under the theme: “From Treatment to Empowerment”.
In this conference, we do not speak about patients alone—We speak with patients, from patients, and for patients. We are here to transform pain into expertise, suffering into knowledge, and chronic illness into dignified, sustainable living.
Over the coming days, we will explore:
  • Cutting-edge research on diabetes, cardiovascular disease.
  • Success stories from patients who defied their diagnoses to become health coaches.
  • Digital tools shifting follow-up from “every 3 months” to smart daily care.
A Special Focus on Saudi Arabia’s Vision 2030:
  • Population Health: Design Strategies to prevent and Decrease Morbidity, mortality of chronic diseases targeting Saudi families and communities for healthier lives by 2030.
  • Care Model Transformation: Implementing the Primary Healthcare-Centered Model (PHC-CM)—shifting 70% of care from hospitals to community clinics, with Empower Team Based Care for personalized chronic disease Screening and management.
  • Health Sector Transformation: Leveraging AI, telemedicine, to deliver equitable, accessible care to every citizen, reducing hospital admissions by 30% and empowering patients as active partners.
We believe every chronic patient is a daily hero,
every doctor a partner in the healing journey,
and every researcher an architect of hope.
Welcome to your second home,
Where science meets humanity,
And hope meets action.
Let’s begin now.. Together.
